Patient Rights

While you are a patient at Alliance Community Hospital, we want you to understand your rights as a patient and how you can help us to take excellent care of you.

Your rights as a patient are to:

  1. Have access to treatment that is medically indicated and available here or at another facility if we do not offer the service, regardless of race color, creed, nationality, religion or the nature of the source of payment.
  2. Be informed about the outcomes of care, treatment, and services that have been provided, including unanticipated outcomes.
  3. Be free from mental, physical, sexual and verbal abuse, neglect and exploitation.
  4. Have your wishes about end of life care addressed.
  5. Have a right to protective and advocacy services.
  6. Be informed of and involved in decisions about care, treatment and services provided .
  7. Have your loved ones and physician notified of your admission.
  8. Have access to the information in your medical record.
  9. Receive care that respects and supports your personal dignity, independence, values and privacy.
  10. Have access to an interpreter if you do not speak English or if your hearing is impaired.
  11. Refuse treatment to the extent permitted by law.
  12. Know the identity and professional role of any individual providing care to you.
  13. Expect care to be given by professionals who listen to your reports of pain and help to relieve it.
  14. Expect that information concerning your care will be kept confidential unless it is your wish that information is shared.
  15. Have resolution when you have concerns about the quality of care including your bill, providers, issues of confidentiality or other concerns.

For concerns about Medicare Providers, call the national number at 1-800-633-4227 or the Ohio Beneficiary hotline at 1-800-589-7337.

If you are having a problem with a HFAP accredited healthcare organization regarding the quality of care you or a loved one has received, the first step would be to contact the healthcare organization and speak with the "patient representative." If you are unable to resolve or diffuse the issue and want to take further action, you need to contact the Quality/Patient Safety Services Department at HFAP.

Your responsibilities as a patient are to:

  1. Provide complete and accurate information about your past, present, and developing health conditions.
  2. Discuss pain relief with your doctors and nurses and ask for pain relief when pain begins or if your pain has not been relieved.
  3. Follow the treatment plan recommended by your physician and be accountable for your actions if you refuse treatment or do not follow instructions.
  4. Be considerate of the property and rights of other patients and hospital Colleagues and be responsible for your own belongings.
  5. Assure prompt fulfillment of financial obligations related to your health care.

ACH Auxiliary Awards Scholarships

The Alliance Community Hospital Auxiliary recently awarded three area seniors the Dr. James J. Thomas Scholarship. This $1,500 scholarship is awarded annually to three recipients who are interested in entering the medical field.

Shape up your Sinuses

April 16, 2014

Dr. David Kanagy, an otolaryngologist with the Alliance Community Medical Foundation, is offering sinus sufferers some relief using an in-office balloon sinuplasty procedure. In the last year, Kanagy became one of the only doctors in a six-county area to begin doing the procedure in the office under local anesthesia, which means no sedation, no IV, no gowns, and no visit to the hospital's surgical department.

ACH enters agreement with Foundation Radiology Group

To provide a consistently high level of care delivery, Foundation Radiology Group and Alliance Community Hospital have entered into a partnership to provide diagnostic imaging services to patients and physicians in Alliance and surrounding areas. Foundation Radiology brings the expertise of a large, highly skilled team of imaging experts to community hospitals across the area. By partnering with Foundation, these hospitals are able to rapidly expand the capabilities of their imaging team in order to keep their patients local.
