To obtain copies of your medical records, contact the Medical Records Department at ACH at 330-596-7163. If you are interested in obtaining copies of your medical record, please call 330-596-7163. If you need X-ray films, which your physician may request, please call 330-596-7700. Copies of medical records sent directly to a physician are provided free of charge, however, there is a nominal charge for copies provided for personal use. |
