Planning for your discharge begins at the time you are admitted to the Hospital. Throughout your stay, your physician and other members of your health care team will discuss your discharge plans with you and your family.
On the day of your discharge, we will facilitate the process so that you can return home as early as possible. However, this process often involves several Hospital departments, therefore we ask for your patience in the event of a delay.
The nursing staff will provide you with any instructions you will need at home concerning special procedures, activities, diets, medications and follow-up visits with your physician.
You will need to arrange for transportation home. If you need referrals to community agencies or extended care facilities, please contact our Social Services Department by calling 330-596-7260.
Alliance Visiting Nurse Association & Hospice, as well as other home care agencies, are also available to provide care as needed at home. Discuss your home care concerns with your nurse or physician.
