The brand new state-of-the-art facility features the Therapy Transitional Trail, a unique rehabilitation area designed to help patients rebuild independent living skills, such as getting in out of a car and grocery shopping, to be able to return home as soon as possible.
Rehabilitation and Residential Facility
The Community Care Center is a 68-bed, extended-care facility that provides a home-like atmosphere and a competent and caring staff to its patients and residents.
Individualized interdisciplinary care is provided encompassing a wide variety of patient needs. Some of these needs include specialized rehabilitation services, complex medical care, convalescent care and end-of-life care. The Center offers three levels of care; transitional, skilled and intermediate care. The Community Care Center is a department of Alliance Community Hospital, offering residents added conveniences such as pharmacy, lab, radiology and emergency services.
CCC implements many of the Planetree ideals into daily activities for patients, such as laughter therapy, aviaries, children’s visits, church services, spiritual resources, plus much more. Friendly, caring staff members also plan various social and recreational activities for patients to participate in throughout the week.
