Hospice

Through their hospice care program, the Alliance Visiting Nurse Association & Hospice knows how to meet the needs of terminally-ill patients and their family members.

We Can Help

Hospice provides comfort, care and compassion for those who are terminally ill.

Alliance Visiting Nurse Association & Hospice is a non-profit organization that recognizes that both the patient and their families are involved in this difficult decision of choosing hospice care. The focus of this Medicare- and Medicaid-certified program is to maximize the quality of remaining life for the patient and family. This is completed by delivering comprehensive services that meet the physical, social, emotional and spiritual needs of the patient and their loved ones.

Services

We offer services that provide assurance, peace and confidence. Services are provided in the home and inpatient settings. Nursing care is provided on an intermittent basis and a nurse is available 24 hours a day for questions or problem solving.

Physician Services

Hospice coordinates patient care through the patient's physician and hospice medical director.

Pain Management

Hospice provides state-of-the-art pain and symptom control.

Financial Support

Hospice pays for medications and equipment related to the terminal illness.

Routine Care

Hospice provides intermittent visits to the patient’s place of residence as determined by the hospice team.

Respite Care

Hospice care provided through an inpatient facility up to five days to allow the caregivers respite.

Continuous Care

Hospice provides 24-hour, in-home care during a crisis.

Inpatient Care

Hospice care can be provided through an inpatient facility for symptom and/or pain control.

Bereavement Care

Hospice can provide counseling for up to 13 months for the surviving family.

Nursing

  • Skilled nurse provides home visits
  • Educational and training of family
  • Coordinates physician ordered care
  • Pain and symptom management

Home Care Aide

Assist patients with personal care needs and maintenance of a clean, safe and comfortable environment

Social Work

  • Clinical intervention for emotional and social needs
  • Coordination of community resources

Clergy

  • Patient's spiritual leader remains involved
  • Other spiritual support is available

Therapies and Volunteers

  • Physical, occupational, speech, massage and dietary
  • Volunteer support services are available to the patient and family for respite and visitation

When a patient's condition is determined to be terminal, with months rather than years of life expectancy, a request for Hospice Care is appropriate.
