Case Management

Alliance Community Hospital’s Case Management Department is a primary resource for our patients that utilizes a team approach and coordinates with the bedside staff to provide quality care that is patient and family-centered, outcome-oriented and resource efficient across the health care continuum.

The Case Management social workers work with patients to make the necessary arrangements for any equipment and community services they may require after discharge.

The RN case managers follow a patient’s stay at ACH and assists them after they are discharged. They discuss the patient’s case with physicians, aid in financial planning, offer services for support groups, and refer patients to Social Services if necessary.
